Patients with diagnosed treatment will be provided a treatment plan and verify estimated insurance benefits upon request. This will give you the opportunity to plan in advance for your dental care and avoid surprises. We ask that co- payment is made at the time of service unless other arrangements are made.
For your convenience, we provide several payment options. We accept cash, check, Visa, MasterCard and American Express. We also offer CareCredit to finance the care you need with No interest payment plans.
As a courtesy, we are happy to submit dental claims on our patient’s behalf. It is your responsibility to give us updated complete insurance information each time your insurance may change. Any treatment recommended by Dr. Gee or Dr. Markuson is indicated regardless of insurance benefits, deductibles, limitations or maximums. We will verify estimated insurance benefits prior to treatment upon request. Please remember that your insurance is a contract between you, your employer and the insurance company. It is to your advantage to know & understand your coverage as not all services are a covered benefit in all contracts. Please contact our office for Insurance details. If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.
